What to Bring to Your First Visit
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Your insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.

From the North (Salmon Creek)
- Start in Salmon Creek: Begin at the intersection of NE 134th Street and NE 20th Avenue.
- Head south on NE 20th Avenue: Travel approximately 1 mile to reach NE 129th Street.
- Turn left onto NE 129th Street: Continue east for about 0.5 miles; the destination will be on your right at 2205 NE 129th Street.
From the South (Portland, Oregon)
- Start in Portland: Begin at the intersection of NW Lovejoy Street and 13th Avenue.
- Head north on NW 13th Avenue: Travel approximately 0.5 miles to reach the I-5 North on-ramp.
- Merge onto I-5 North: Continue on I-5 North for about 5 miles, crossing the Columbia River into Washington.
- Take exit 1C for NE 134th Street: After crossing into Washington, take exit 1C toward NE 134th Street.
- Turn left onto NE 134th Street: At the exit ramp, turn left onto NE 134th Street.
- Turn left onto NE 20th Avenue: After approximately 0.2 miles, turn left onto NE 20th Avenue.
- Continue south on NE 20th Avenue: Travel approximately 1 mile to reach NE 129th Street.

From the East (Camas)
- Start in Camas: Begin at the intersection of NE 3rd Avenue and NE Dallas Street.

From the West (Banks)
- Start in West Linn: Begin at the intersection of S Willamette Falls Drive and S 7th Place.
- Head north on S 7th Place: Travel approximately 0.2 miles to the intersection with Willamette Falls Drive.
- Take the ramp onto I-205 North: Merge onto I-205 N and travel for about 6 miles.
- Take exit 27 for NE 134th Street: After crossing into Washington, take exit 27 toward NE 134th Street.
- Turn left onto NE 134th Street: At the exit ramp, turn left onto NE 134th Street.
